Does Varicocele Cause Infertility?

Varicocele, which occurs with the enlargement of the veins that supply the testes, is one of the most common urological conditions in men. One of the most important questions is “does varicocele cause infertility?” The short answer: Varicocele does not cause infertility in every man, but it is one of the most important and treatable causes of male infertility.

How Does Varicocele Cause Infertility?

Varicocele, as a result of the enlargement of the veins around the testis, causes blood to flow back and the testicular temperature to increase. The testes should normally work in an environment cooler than body temperature. An increase in temperature has a negative effect on sperm production.

The effects of varicocele on sperm are:

  • A decrease in sperm count
  • A decrease in sperm motility
  • An increase in sperm shape abnormalities
  • An increase in sperm DNA damage

These changes can reduce male reproductive capacity over time.

Does Every Man with Varicocele Become Infertile?

No. Varicocele:

  • Gives no symptoms at all in some men
  • Sperm values may be completely normal
  • May not affect the ability to have children

In other words, the presence of varicocele on its own does not mean infertility. What matters is whether the sperm parameters are affected.

In Whom Does Varicocele Cause Infertility?

The risk of varicocele causing infertility is higher in certain situations:

  • Long-standing varicocele
  • Advanced (Grade 2 to 3) varicocele
  • Patients with deterioration in the sperm analysis
  • Repeated inability to have children
  • Patients who develop shrinkage of the testis (atrophy)

By Which Mechanisms Does Varicocele Affect Fertility?

The main effects of varicocele are:

Increase in Temperature

Due to the enlarged veins, the testicular temperature rises and sperm production is impaired.

Oxidative Stress

Increased free radicals can damage sperm DNA.

Hormonal Effects

As testicular function weakens, testosterone production can also be affected.

If Varicocele Is Treated, Does Infertility Improve?

In many patients, yes. Especially in suitable patients, after varicocele surgery:

  • Sperm count can increase
  • Sperm motility can improve
  • The chance of pregnancy can increase

However, the same success is not achieved in every patient. The best results are seen in:

  • Those whose sperm values are impaired
  • Young patients
  • Those treated in the early period

.

Does Varicocele Surgery Increase the Chance of Pregnancy?

Scientific studies show that in suitable patients, varicocele surgery:

  • Can increase natural pregnancy rates
  • Can improve sperm parameters

. However, the success rate varies from person to person.

How Is Varicocele Diagnosed?

The diagnosis is usually made with:

  • Physical examination
  • Scrotal Doppler ultrasound
  • Sperm analysis (spermiogram)

.

When Is Treatment Required?

Treatment is considered in the following situations:

  • Inability to have children
  • Deterioration in the sperm analysis
  • Pain in the testis
  • Shrinkage of the testis

Summary

Varicocele does not cause infertility in every man; however, it is one of the most important and treatable causes of male infertility. It can negatively affect reproductive function, especially in cases where it impairs sperm quality. With correct patient selection and appropriate surgical treatment, sperm values can improve and the chance of pregnancy can increase in many men.
