What Does an Elevated PSA Mean?

PSA (Prostate Specific Antigen) is a protein produced by the prostate gland that can be measured in the blood. The PSA test is one of the most important laboratory tests used to evaluate prostate health. It plays an important role especially in prostate cancer screening and in the evaluation of benign prostatic hyperplasia and prostate infections. However, an elevated PSA on its own does not mean a diagnosis of prostate cancer. For this reason, a high PSA result must always be evaluated in detail by a urology specialist.

Conditions That Cause an Elevated PSA

There are many conditions that cause the PSA value to rise. Benign prostatic hyperplasia (BPH), prostate inflammation (prostatitis), urinary tract infections, difficulty urinating, retention of urine in the bladder, and mechanical stimulation of the prostate can raise the PSA level. In addition, a recently performed prostate examination, cystoscopy, catheter placement, intensive cycling, or sexual activity can also cause a temporary rise in PSA.

Evaluation of an Elevated PSA

When an elevated PSA is detected, the first stage is a detailed evaluation. The patient’s age, family history, medications used, prostate volume, and current complaints are taken into account. A digital rectal examination of the prostate may then be performed, and when necessary, a free PSA test, a PSA density calculation, or an evaluation of the PSA rate of rise may be requested.

Multiparametric Prostate MRI

Today, one of the most frequently used advanced examinations in patients with an elevated PSA is multiparametric prostate MRI. This imaging method allows suspicious areas within the prostate to be detected and provides important information about the need for a biopsy. The MRI examination usually takes 30 to 45 minutes and does not require a hospital stay.

Prostate Biopsy

If suspicious findings are detected as a result of the MRI, a prostate biopsy may be recommended. The biopsy procedure is usually performed under local anesthesia or sedation. During the procedure, samples are taken from the prostate tissue and sent for pathological examination. The biopsy takes about 15 to 30 minutes, and patients can most often return home the same day. Pathology results are usually available within 5 to 10 days.

Treatment of an Elevated PSA

The treatment of the condition causing the elevated PSA varies according to the underlying disease. If the cause is a prostate infection, antibiotic treatment and supportive medications are administered. The duration of treatment can vary between 2 and 6 weeks in most patients. After treatment, the PSA test is repeated to assess whether the level has returned to normal.

If the cause of the elevated PSA is benign prostatic hyperplasia, medication treatment may be started. These treatments aim to ease the urine flow and reduce the prostate volume. In patients who do not benefit sufficiently from medication or who have advanced prostate enlargement, laser prostate surgery, closed prostate surgery, or robotic surgery methods can be applied. The majority of patients are discharged within 1 to 3 days after these procedures and can return to their daily lives in a short time.

If a diagnosis of prostate cancer is made as a result of the examinations performed, the treatment plan is determined according to the stage of the disease, the patient’s age, and general health condition. In early-stage prostate cancers, options such as active surveillance, robotic prostate surgery, or radiotherapy can be considered. After robotic prostate surgery, patients usually stay in the hospital for 1 to 2 days, while the time to return to daily life varies between 2 and 4 weeks in most patients. Radiotherapy treatments are mostly applied in sessions lasting several weeks.

Follow-up and Monitoring

Not every patient with an elevated PSA needs surgery. Today, many patients can be safely monitored with regular follow-up programs alone. During the follow-up process, PSA measurements are taken at certain intervals, MRI imaging is repeated when necessary, and the patient’s clinical condition is evaluated.

Regular urological check-ups are recommended especially for men over the age of 50. In people with a family history of prostate cancer, screening may need to be started at an earlier age. Thanks to early diagnosis, both the treatment success of prostate cancer increases and the complaints that impair quality of life due to prostate enlargement can be effectively brought under control.
