Benign Prostatic Hyperplasia (BPH)

BPH (Benign Prostatic Hyperplasia) is the benign enlargement of the prostate gland. It is not cancer; however, because the prostate surrounds the urinary channel, as the enlargement progresses it can disrupt urine flow and lead to serious quality of life problems.

Symptoms

BPH usually progresses slowly, and complaints increase over time:

  • Frequent urination
  • Getting up at night to urinate (nocturia)
  • Weak and thin urine stream
  • Delay in starting urination
  • A feeling of not being able to fully empty the bladder
  • Dribbling after urination ends

Who is it more commonly seen in?

  • Especially in men over the age of 50
  • Its frequency increases with advancing age
  • It is related to hormonal changes and the growth of prostate tissue

Diagnosis

The diagnosis of BPH is made through clinical evaluation and certain tests:

  • Physical examination (rectal examination)
  • PSA test
  • Prostate and bladder ultrasonography
  • Urine flow test (uroflowmetry)
  • Post-void residual measurement if necessary

Treatment options

Treatment is determined according to the severity of the disease and its effect on quality of life:

  • Medication treatment: medications that relax or shrink the prostate
  • Minimally invasive methods: interventions such as laser or vaporization
  • Surgical treatment
    – TUR-P (closed prostate surgery)
    – HoLEP (laser prostate enucleation)

Today, closed and laser-based methods in particular stand out with their high success and low complication rates.

Frequently Asked Questions

No. BPH is a benign disease and does not turn into prostate cancer. However, both diseases can be present together.

No. In mild and moderate cases, medication treatment is sufficient. However, when there is advanced obstruction and quality of life deteriorates, surgery is required.

In advanced cases, urinary retention, kidney involvement, and recurrent infections can develop.

Clinical Summary

BPH is a common prostate disease that increases with age and lowers quality of life by mechanically blocking urine outflow. With early evaluation and appropriate treatment, complaints can largely be brought under control.
