This text has been prepared to inform you about the processing of the personal data you share through the appointment and contact form on our website, within the scope of the Personal Data Protection Law No. 6698 (“KVKK”).
1. Data Controller
Your personal data is processed by Prof. Dr. Abdulmuttalip Şimşek (Ataköy 1. Kısım Mah. Yeni Fişekhane Cad. Ataköy 1. Kısım G59 No:1 İç Kapı No:3 Bakırköy / İstanbul) in the capacity of data controller.
2. Personal Data Processed
Through the form, identity and contact data (full name, phone number) and health data are processed. The “topic of interest” you select in the form (robotic surgery, prostate, kidney, urethra, men’s sexual health, etc.) falls within the scope of special categories of personal data, as it contains information about your health status.
3. Purposes of Processing Personal Data
It is processed for the purposes of receiving your appointment and preliminary consultation request and contacting you, providing information appropriate to your request and planning the process, and providing guidance appropriate to your request.
4. Legal Basis
Your full name and phone data are based on the legitimate interest in contacting you and meeting your request, and on the legal grounds of establishing a contractual relationship (KVKK art. 5). Your health data is processed only with your explicit consent, in accordance with KVKK art. 6.
5. Transfer of Personal Data
Your personal data may be shared domestically, limited only to the above purposes, with the clinic/secretarial staff assigned to carry out the service and with the health institutions cooperated with. Your data is not transferred abroad and is not shared with third parties for marketing purposes.
6. Method of Collecting Personal Data
Your personal data is collected electronically through your completion of the form on the website.
7. Your Rights Under Article 11 of the KVKK
By applying to the data controller, you have the right to learn whether your data is being processed, to request information, to learn the purpose of processing and whether it is used in accordance with its purpose, to request the correction of your data if it has been processed incompletely or incorrectly, to request its deletion or destruction if the reasons for processing have ceased to exist, and to request compensation for damages arising from unlawful processing.
